In their own words
Microsoft Outcome Management
Microsoft Outcome Management is a free, open-source template app for nonprofits, built on Microsoft Power Platform. It helps teams set program goals and indicators, measure results, and aggregate impact across programs, projects, and activities. Users can maintain individual and organizational constituent records, including participants and stakeholders, with demographic, relationship, contact, and communication-preference details. The app follows work from strategic objectives through stakeholder measurement and supports outputs, outcomes, results, and indicator values recorded as baseline, actual, or target measurements over time. Its foundation is the open-source Common Data Model for Nonprofits, intended to support interoperability and data transparency. The maker describes connections with other applications through Power Platform data flow. Deployment requires installing that data model first, then building and importing the solution into Dataverse with .NET and Power Platform CLI. The listed build requirements include .NET SDK 4.6.2 or later, current .NET and Power Platform CLI, Node.js 18.18.0 or later, and suitable Dataverse permissions. Platforms are web and API.
