In their own words
AgreeDock
AgreeDock is a web service for creating digital waivers, consent forms, and rental agreements. Businesses can describe a form for AI to create or begin with a template, then add questions, terms, business details, and branding. Forms are shared through links or QR codes. Customers sign in a browser on a phone, tablet, or computer, or sign in person, without downloading an app. Businesses can see who signed, search customer records, and download completed waivers as PDFs. A PDF may include the waiver text, responses, signature, signing date, and document version, while each signed record preserves the version accepted. AgreeDock is presented for tattoo and piercing studios, gyms, fitness and beauty businesses, yoga and Pilates studios, camps, retreats, activity operators, and rental businesses. The Free plan costs nothing and allows 10 signatures monthly and one active waiver. Pro is listed at $10 a year, with paid pricing from 15 /mo.
Survtapp
Survtapp collects waivers, surveys, checklists, and event leads on iOS and Android, including when devices are offline. Responses, signatures, and photos are queued locally and sync when connectivity returns. Its form builder offers 22 question types, templates, signatures, photos, voice notes, and conditional logic. Kiosk mode can lock an iPad or Android tablet to one form. Survey responses can be exported as CSV, XLSX, JSON, or PDF. Native integrations are listed for SendGrid, Mailchimp, GoHighLevel, HubSpot, and Constant Contact; webhooks use HMAC-SHA256 signatures, and REST API keys are available on paid plans. The maker says database and file storage are hosted in AWS Canada’s Montréal region and that the service aligns with PIPEDA and GDPR. The free plan costs nothing forever and includes 3 users, 100 responses per month, 5 MB storage, 2 kiosk devices, 3 forms, and 120-day retention. Paid plans start with Solo at $19 a month; higher plans add capacity, retention, integrations, or support.
