Survtapp
Survtapp collects waivers, surveys, checklists, and event leads on iOS and Android, including when devices are offline. Responses, signatures, and photos are queued locally and sync when connectivity returns. Its form builder offers 22 question types, templates, signatures, photos, voice notes, and conditional logic. Kiosk mode can lock an iPad or Android tablet to one form. Survey responses can be exported as CSV, XLSX, JSON, or PDF.
