CliniForm combines bookings, client records, forms, treatment notes, communication, payments, and practice management in one platform. It is aimed at independent wellbeing professionals and small teams such as therapists, trainers, beauticians, and clinic owners. Clients can book online, while the smart calendar manages availability, practitioners, rooms, and appointments. Consultation and consent forms support digital signatures and treatment-plan consent workflows. Client records can include consultations, risks, treatment history, SOAP notes, payments, and progress. A unified inbox links SMS, email, and WhatsApp conversations to client records; messaging usage charges may apply separately. Listed integrations include Google Drive, OneDrive, Dropbox, Make, Zapier, WordPress, and WhatsApp Business. CliniForm also provides a REST API and webhooks, with practice-scoped API keys on Team and Enterprise plans. The company says data is hosted in the UK and clinical files are encrypted with AES-256.
